The Woman's Health Record serves as a complete record for a woman's gynecologic care. It includes forms for recording history, physical examination, medical decision making, problems, immunizations, and routine and high-risk laboratory services. The record has been designed to aid in documenting and correctly coding women's health care services and is compliant with the most current (1995 and 1997) Medicare Documentation Guidelines. A table of high-risk factors that indicate the need for specific services also is included.
